Welcome to the Restorative Dental Group of Cambridge!

On your first visit, you will meet either your doctor or hygienist and have a thorough examination of your teeth and gums. You will have x-rays and/or a comprehensive dental cleaning, as needed. During the visit, we will also ask you to fill out a form to become familiarized with your medical and dental histories.

For your convenience, below are two of the forms we will ask you to fill out on your first visit. If you would like, you can complete them ahead of time.
